INSTRUCTIONS FOR INSTALLATION OF HELIOS
COVERS FIXTUR~ 5412 and 5413
. FOR OCTAGONALOUTLET BOX MOUNTING.
1. ScrewCOVER PLATEtothe backsideof MOUNTINGPtATE withscrewsprovided.
2. Pullsupplywires from outlet box through tha WIREWAY HOLE in MOUNTING PLATE.
3. Using outlet box screws (provided with outlet box) secure MOUNTING PfATE to outlet box using the appropriate SLOTS in MOUNTING
PIATE.
4. Using the HOLES next to each MOUNTING STUD on MOUNTING PLATE secura MOUNTING PLATE directly’to wall using wood scfews
toggle bolts, or alternate fastener (not provided) depending on wall material.
. FOR HORIZONTALLY MOUNTED RECTANGULAR GEM OR SWITCH BOXES. (Diecard Coverplate)
i. Pull supply wires from outtet box through the WIREWAY HOLE in MOUNTING PLATE.
2. Using outlat box acrewa (providad with outlet box) secure MOUNTING PLATE to outlet box using the appropriate SLOTS in MOUNTING
PL4TE.
3. Using tha HOLES naxi to aach MOUNTING STUD on MOUNTING PLATE secure MOUNTING PLATE directly to wall using wood
screws, toggle bolts, or alternate fastener (not provided) depending on wall material.
