Trane TWH030C100A5 has a vibration when the fan is...

Trane TWH030C100A5 has a vibration when the fan is running. I suspect the bearings are shot. The unit was installed 10/23/2001, and is either in Cool or Heat year around. I cannot find a tag on the motor. Please help define a part # so I can obtain a new motor. Thanks, Bob bob28405@bellsouth.net
